2015 Annual Conference Evaluation and Evidence of Completion Form

The 2015 Annual Conference evaluations were sent electronically on Friday, May 1 and again on Monday, May 18. You need to complete this evaluation in order to receive an emailed link to the Evidence of Completion (EOC) form, which has replaced the previous certificate of attendance. The EOC link was sent on Friday, May 15 and will be sent again on Friday, May 29 to those who had not completed the evaluation by May 15.

Unfortunately, the EOC form that we originally posted was missing our signature and some other information. We apologize for this error. We have since replaced the document and will send an email with a link to the corrected version of the form to those who had previously completed the evaluation survey. Those attendees who complete the evaluation between May 15 and May 29 will be emailed a link to the correct EOC form on May 29.

We are currently in the process of compiling the total credits earned by all attendees of our Annual Conference into our database. This information should be available to view within the next few weeks by logging in to our website, clicking on the “My Professional Dev” in the left-hand frame, and then scrolling down to the bottom of the screen.

Any licensed professional who would like to report PD Clock Hours earned from Annual Conference to the Illinois State Board of Education (ISBE), must enter the credits themselves into the ELIS system by June 30, 2015 in order to receive credit.
